Comfort Keepers—a non-medical in-home care company servicing Clark, Champaign, Logan, Montgomery, Greene, Miami, Shelby, Darke and Preble Counties—
is looking for RN Client Care Coordinators (RN CCC) interested in helping seniors remain independent in their homes.
This position ensures the continuity of care for clients after referral. The RN CCC facilitates the provision of in-home care and support services to our clients, performs initial and follow-up client assessments, develops the Plan of Care, and arranges for uninterrupted client care. The RN CCC routinely interacts with other personnel involved in the client’s care to include family members, community providers, referral sources, insurance companies, care managers, and other third party payors.
Qualified applicants must possess a current RN license with at least two years experience in a health care setting providing direct patient care. Experience in admission and discharge planning a plus. Must possess and demonstrate excellent communication skills and initiative, as well as a positive, professional business image. Valid driver’s license, auto insurance, and a favorable background check required.
