UNDERSTANDING REHABILITATION CARE With shrinking budgets, hospital stays are often shorter than they were in the past, which means sometimes people are not well enough, strong enough or functional enough to go straight home after leaving the hospital.  Most people are anxious about returning home after a hospitalization, even if they’re told they can do their therapy at home.  Returning home before you are ready can lead to falls, re-injuries or new complications.  It can also mean that you may require 24-hour care within your home.  This type of care can be less effective and more expensive than a period of rehabilitation with a professional, coordinated rehab team in a licensed post-acute facility like Heritage Hall.

  • SKILLED CARE – When people use the term “skilled care”, they are usually referring to some combination of rehabilitation and skilled nursing
  • EXTENDED CARE – We’re proud to say that more than 80% of our patients return home after using our rehabilitation and skilled nursing services.  But sometimes it’s not possible to receive all the care they need at home.  That’s why Heritage Hall is dedicated to also providing excellent care to patients requiring ongoing assistance with the routine activities of everyday life.
  • HOSPICE CARE – We understand no one wants to experience the end-of-life process alone.  Our goal is to provide the best quality of life in hospice care through giving spiritual, mental, emotional and physical comfort to our patients, their families and other caregivers.
  • RESPITE CARE – Respite Care is a short-term residential care solution to help with a loved one while their primary caregiver is unavailable or simply needs a period of rest.  During this time, Heritage Hall provides the same services that are available to all of its residents, including tailored therapies, diet and social activities.
