Liberty Commons Rehabilitation Center has served Wilmington, North Carolina and surrounding areas since 1994 by providing a complete continuum of care, including short-term rehabilitation, skilled nursing, long-term living, assisted living, outpatient therapy, hospice care, palliative care and respite care.
When you arrive at Liberty Commons, you are greeted by a clinical team who develops a person-centered care plan designed to help you regain skills and decrease the chance for readmission back to the hospital. You also have access to medical services that address secondary conditions for a complete treatment plan.
